Creating your own cookbook?

For Christmas my mother simply asked for a "book" in which she could store her recipes. I researched expensive pre-organized books with color coded index cards and a lot of fancy extras that she may not need. I also I considered an inexpensive notebook filled with clear sheet protectors but that seemed to plain and cheap. I'd like a nice combination of both. Has anyone created their own cookbook heirloom/purchased a "fill in the blank" cookbook? Any suggestions would be greatly appreciated.
